Delhi Metro, Gammon spar on crane mishap inquiry report

The inquiry report into the July 12 accident at the Metro construction site inZamrudpur that killed six workers and left 15 others injured was placed before Delhi Metro chief E Sreedharan late Thursday evening.
While that report is under examination by Metro’s board of directors, the report on the July 13 crane mishap at the same site while removing debris has got the Delhi Metro Rail Corporation (DMRC) and contractor for the line, Gammon India Limited, sparring.
On July 13, four cranes were lifting the launching girder that had collapsed the day before when the boom of one snapped. As a result, two adjoining cranes gave way and the third toppled. Six workers received minor injuries, and three cranes were destroyed in the event.
The inquiry report on this mishap by B P Singh, an expert on construction of heavy structures, says, “Mechanical failure of cranes is primarily due to sudden overloading of cranes as well as side thrust due to non-correction of swing”. The report (Newsline has a copy) also says, “In this situation, team work demands free interaction with experienced operators, lifting engineers and managers for giving ultimate plan of operation.”
While DMRC managing director Sreedharan says site-clearing operations were carried out by contractors and any flaw in crane operations will be their responsibility, senior Gammon officials said DMRC was consulted throughout the process.
Gammon is responsible for site-clearing operations as per the contract.
But the probe report says interactions were also held with DMRC’s chief project manager (south-east) S K Gupta, Gammon vice-president (plant) V K Luthra, and Gammon’s deputy project manager Manish Dutta.
Also present at the site were other senior officials from contractors like CEC SOMA, ALLCARGO, and Amrik Singh and Sons Cranes, the report says.
Sreedharan said, “Proper and safe method of lifting should have been adopted by Gammon. The entire crane operation and recovery was being done by the contractor and DMRC engineers are not experts in crane operation.”
He said DMRC engineers were involved “only in assisting Gammon in procuring cranes”.
But Umesh Gupta, assistant vice-president of Gammon India, asked, “Why is the contractor held responsible for everything? Senior DMRC officials were present during the operations and were consulted at every step. Even they should take responsibility for the ncident.”
Senior Gammon officials also alleged that contract terms with DMRC are one-sided and nails a contractor for all loopholes despite DMRC being the supervising and approving agency for all designs and construction work.

Sreedharan apprises Board of Directors of mishap report
NEW DELHI: The Board of Directors of the Delhi Metro Rail Corporation met on Monday to discuss the report submitted by the Nagpal Committee on the mishap in Jamrudpur here on July 12 that claimed six lives.
At the meeting held in Metro Bhavan here, DMRC Managing Director E. Sreedharan apprised the Board of Directors of the report which was submitted to him last week.
According to sources, action has been initiated against a few DMRC officials following the report. However, a Delhi Metro spokesperson declined to authenticate this.
We cannot deny or accept the report. The findings of the Committee will be announced by Dr. Sreedharan on Tuesday, he said.
The report was filed by the Committee comprising Prof. A. K. Nagpal of IIT Delhi, Prof. B.R. Bose of Delhi College of Engineering and Steven Lowry, Project Director, General Consultants, DMRC. Rajan Kataria, the fourth member of the Committee, was dropped a few days ago.
The report, which was submitted to the DMRC this past week, was first studied by the Directors of the Corporation and later submitted to Dr. Sreedharan.

Wednesday, Jul 29, 2009 Delhi Metro will be on guard: Minister NEW DELHI: Union Urban Development Minister S. Jaipal Reddy assured the Rajya Sabha that the Delhi Metro Rail Corporation would henceforth be on guard to avoid accidents such as the one that occurred on July 12 which lead to the loss of six lives.
Assuring that all cantilevers were being thoroughly checked, Mr. Reddy allayed fears about the running of Delhi Metro, drawing a distinction between accidents during construction and those that take place during the actual running of trains.
Distinction blurred
We need to understand that accidents that occur at construction stage are qualitatively different for those taking place during train operations. We are satisfied that in the last seven years of operating stage I of the Delhi Metro, DMRC has not had one major accident. All kinds of fears are being raised and the distinction is being blurred, he said, replying to clarifications sought by the members, led by Leader of the Opposition Arun Jaitley.

NEW DELHI: The probe team set up by the Delhi Metro Rail Corporation has attributed the July 12 mishap at Jamrudpur in South Delhi that claimed six lives to deficiency in design and lack of adequate curing of concrete.
Making public the findings on Tuesday, DMRC Managing Director E. Sreedharan pinned the blame on the design team, and held incorrect advice by structural consultants as responsible for the tragic mishap: The report of the committee, comprising A. K. Nagpal of IIT Delhi, B. R. Bose of Delhi College of Engineering and Stephen Lowry of General Consultants, was examined by our Board of Directors and has been accepted in toto. The findings reveal a major deficiency in the design of the cantilever arm. The second fault was that the concrete used did not have adequate strength, probably due to lack of adequate curing. Explaining the fault in design, Mr. Sreedharan said: The reinforcement steel was not provided at critical places. A cantilever must have reinforcement steel at critical places. At certain tension points it was not properly distributed.
The committee concluded that concrete was well compacted, that there was no visual evidence of voids or irregularities, and that the reinforcement spacing and lapping was generally in accordance with the design drawings. However, based on tests carried out on cores taken from the pier and cantilever, it was found that the concrete failed to meet the strength criterion by a wide margin.
The committee said the cement content was only marginally lower than required in the design mix, and the reinforcement steel was of the required quality.
The committee feels the lack of curing might be the reason for the low concrete strength, said Mr. Sreedharan.
Consultants misled us
Blaming Tandon Consultants for misleading the DMRC about the cracks that appeared on pier No. 67, designed by Arch Consultancy Services, Mr. Sreedharan said these were first noticed on April 1, and on April 3.
Tandon Consultants were asked for advice. In their report submitted on April 15, they said the cracks cannot be attributed to inadequacies in design or material. They said the cracks were due to the settlement of concrete and misled us into carrying on with civil works, he added.
Referring to action being taken by the DMRC against the erring officials and the design and structural consultants, he said: We have decided to blacklist Arch Consultancy Service for five years, Tandon Consultants will be debarred for two years, and Gammon India, the contractor for the line, will be issued a show-cause notice as to why they should not be blacklisted for two years.
Deputy chief engineers suspended
Taking action against its own employees, the DMRC has suspended deputy chief engineers V. P. Srivastava and Mukesh Thakur who were directly responsible for the design and site supervision. Chief engineer (Design) Rajan Kataria will be served with a major penalty charge sheet.
The Corporation has also decided to demobilise and replace the expatriate consultant of General Consultants, C. Mallonga, who was directly responsible for ensuring the quality of work.
The DMRC has begun rechecking all cantilever piers to assess the quality of work at the rest of Phase II constructions.
There are 87 cantilever piers, of which 38 are along the airport line. They will be rechecked by Shirish Patel and Associates. Based on their advice, necessary action will be taken to ensure that all cantilever piers are safe, Mr. Sreedharan said. Shirish Patel and Associates have been asked to examine the 18 cantilever piers that have been found with hairline cracks.
Each and every pier will now be examined minutely and meticulously. The DMRC will not rely entirely on its own design team; there will now be checking at two levels. First, independent consultants will check the design, and then the DMRC design office will check them, Mr. Sreedharan said.
The DMRC has also decided to get all designs checked in future by General Consultants or by an outside consultancy, and strengthen its own design wing.

NEW DELHI: With the Delhi Metro probe panel finding serious fault with the design and material that led to a major accident at a construction site July 12, the Delhi Metro Rail Corporation (DMRC) has decided to blacklist its design consultants and crack down on some senior officials, urban development minister S Jaipal Reddy informed the Lok Sabha Tuesday.
"The DMRC has taken 10 steps in the wake of the Metro mishap and to ensure that such incidents don't re-occur," Reddy said.
He said the design consultant, Arch Consultancy Services, has been blacklisted for five years while the structural consultant, Tandon Consultants, which did not give "correct advice" to DMRC is being debarred for two years.
The contractor Gammon India will be issued a show cause notice for blacklisting for two years, he said.
Reddy said that Vijay Anand, director of DMRC in whose jurisdiction the accident took place, as well as the previous accident involving a launching girder collapse, has been sent back to the railways.
Two deputy chief engineers directly responsible for the design and site supervision, VP Shrivastava and Mukesh Thakur, have been placed under suspension, Reddy informed the house.
Rajan Kataria, the chief engineer design, DMRC, will be issued with a major penalty chargesheet, he added.
The July 12 accident at a Metro construction site in Zamrudpur in south Delhi caused the death of six workers, including an engineer.

Delhi Metro Rail Corporation Chief E. Sreedharan on Tuesday announced that the Metro Rail project will finish its second phase before 2010 Commonwealth Games despite accidents.
On July 12, an overhead bridge under construction gave way and crushed six workers to death and the following day in another incident two giant cranes carrying a huge girder toppled at the same site, leaving five workers as injured.
"This accident will not have any effect on the overall completion of the project in different lines, there is not going to be any change at all. We will still be able to complete our phase II comfortably before the Commonwealth Games," Sreedharan told media in the capital.
After the discovery of about a dozen hairline fractures on cantilever piers at the Delhi Metro's ongoing works across the city, the Delhi metro chief said that henceforth there would be double checks before designs are taken from the drawing board to the site.
"We will get an independent agency to check the designs first. When a contractor's design consultant submits a design, it will be checked by either our own general consultants or we will get an outside consultancy company to check the design first and then pass on to DMRC (Delhi Metro Rail Corporation) design office so that there will be two level of checks before the designs are cleared for execution," said Sreedharan.
Sreedharan blamed "deficiency" in design and material for the July 12 mishap which claimed six lives and claimed that a design consultant firm which inspected the site "misled" them and the contractor about the condition of the pier which collapsed.
He informed that the DMRC was also serving a show-cause notice to contractor Gammon India, which was carrying out the work at Zamrudpur accident site, to explain why it should not be blacklisted for two years, besides suspending its two Deputy Chief Engineers.
It also blacklisted Arch Consultant, design consultant of Gammon India, for five years besides issuing a "major penalty chargesheet" to Chief Engineer (Design) Rajan Kataria.
Delhi Metro chief, making the findings of a probe into the Zamrudpur accident public, said Tandon Constructions were appointed by them to inspect the site after cracks were reported.
"The crack on the pier 67 was first noticed on April one and we approached them on April three. They gave us a report on April 15 saying that there was no inadequacy in the design. The consultant misled the DMRC and contractor,"Sreedharan told a press conference.
The Board of Directors of DMRC which met in New Delhi on Monday to study the report of the three-member panel headed by IIT professor A K Nagpal decided to blacklist Tandon Construction for two years.
Sreedharan said, according to the report, the design of the cantilever pier which collapsed in Zamrudpur was not adequately taken care of.
"There was a deficiency of steel at critical areas besides a major deficiency in reinforcement detailing," he said.

Metro trains delayed
NEW DELHI: Passengers commuting between Dwarka and Yamuna Bank complained of delay in the Metro service on Wednesday. While some claimed that the Metro services were affected for as long as 15 minutes, a DMRC official said the delay was caused by the ongoing checking drive launched after the mishap in Jamrudpur.
The official said an inspection was carried out at a bridge at Pragati Maidan and two trains were halted at the Indraprastha Station instead of being allowed to go to Yamuna Bank.

The Delhi High Court today issued notice to Delhi Metro Rail Corporation (DMRC) on a petition filed by Residents Welfare Associations (RWA’s) of Greater Kailash over its failure to take care of all apprehensions of the residents.
The petition sought to draw the attention of the court to the DMRC’ failure to comply with the assurance given to the court that all apprehensions of the residents, including those regarding safety, would be taken care of while building the elevated line.
A bench comprising Chief Justice A P Shah and Justice Manmohan heard a petition filed by the E-block RWAs of GK-I, who in their petition, said the DMRC should have constructed underground metro instead of the elevated one and in future, they must consider this.
Citing the accident which occurred on July 12 at the under- construction elevated line between Defence Colony and Nehru Place, the residents said overhead constructions of Metro are a health and safety hazard. They cause a loss of environmental aesthetics as well degradation of the fauna.
The residents want that the Court should issue directions to DMRC to work on the underground route in future as the elevated stretch would wreak havoc in the congested area, putting at stake, the safety of the public at large. Besides, they said, it would intrude their privacy and also threaten the area’s green cover.

New Delhi Even as the Delhi Metro faces an acute shortage of skilled labour, its training school located in Shastri Park received a shot in the arm recently through the ISO 9001:2008 certification, a validation from the International Organization for Standardization (ISO), for maintaining a Quality Management System in its training programmes. Established in 2002, the Delhi Metro Training School is the only specialised training institute in Metro operations and maintenance technology in South Asia. The programme was initially developed with the help of the Hong Kong Metro.
The institute has already trained more than 5,000 employees of the Delhi Metro’s Operations and Maintenance (O&M) Wing, CISF and Delhi Police personnel, Bangalore Metro personnel, contractors’ staff (ticketing, customer facilitation, etc), customer relation assistants (CRAs) and other departments of the organisation. “So far this year, 1,160 new staff members have joined the institute and 550 have already undergone training,” Anuj Dayal, DMRC spokesperson, said.
“TUV SUD South Asia Private Limited, a testing and certification body, found that the Delhi Metro Training Institute has established and applies a Quality Management System (QMS) for design, development and delivery of its training programmes. An audit was performed by the body, which found that the institute fulfilled quality requirements according to ISO 9001:2008. All training activities, including the training need analysis, designing module, pedagogy, feedback analysis and evaluation were audited by the certifying body,” Dayal said.
The training school has specialised working model rooms — for understanding the functioning of the signalling and automatic fare collection (AFC) systems.
The institute plans to procure one more simulator by the end of this year so that train operators can have more hands-on experience of driving a Metro train.
The institute’s expertise in training Metro staff is expected to come in handy for other cities like Chennai, Kochi, Bangalore, Mumbai, Hyderabad, Ludhiana and Pune, which are planning to have Metro rail projects in their cities soon.

Delhi Metro records highest ridership PTI 5 August 2009, 07:26pm IST
NEW DELHI: In another first, Delhi Metro has recorded the highest ridership of almost 10 lakh passengers on a single day and earning the maximum revenue on consecutive days.
The number of commuters who took the Metro yesterday stood at 9.92 lakh while the revenue earned was Rs 1.24 crore.
On Monday, the ridership stood at 9.26 lakh and the revenue Rs 1.25 crore, the highest earning so far since the Metro became operational on 25th December 2002.
"According to a commuter survey, 43 per cent of Metro passengers use two or three trains during each trip. This means that the actual train occupancy yesterday was 14.21 lakh," a DMRC spokesman said.
Officials attributed the high ridership to Raksha Bandhan festivities during which people travel from one place to another.
The Metro's Line-3 that connects Dwarka with Yamuna Bank had the maximum ridership at 3.96 lakh, followed by Line-1 (Dilshad Garden-Rithala) at 3.05 lakh and Line-2 (Jahangirpuri-Central Secretariat) at 2.89 lakh yesterday.
Delhi Metro is running 60 extra train trips and has deployed additional personnel today to cater to extra rush on Raksha Bandhan.
The stations which witnessed the heaviest rush today were Yamuna Bank, Rajiv Chowk, Jahangirpuri, Shahdara, Seelampur, Dilshad Garden, Dwarka Mor and Uttam Nagar (East).
To facilitate commuters, manual token counters were opened and 111 extra shifts of ticket machine operators and 56 more shifts of customer facilitation agents operated on Rakhi day.
DMRC's previous highest ever single-day ridership of 9.53 lakh was recorded on November 24 last year during the India International Trade Fair.
In July, the average daily ridership of the Delhi Metro stood at 8.37 lakh, which is the maximum ridership so far during any month.

New Delhi, Aug 5 With Delhi Metro running extra trains today on the occasion of Raksha Bandhan, over seven lakh people opted the new age transport system till evening generating a revenue of Rs one crore for the DMRC.
About 7.36 lakh people travelled on the Delhi Metro till 8 pm on all three lines of the transport system thus generating a revenue of Rs 1 crore.
DMRC officials said the ridership would go high as the number of people travelling would be higher after 8 pm."We expect to generate more revenue today on Raksha Bandhan. The rush is mostly on the long-distance routes,"they said.
The officials said since people go back their homes after meeting their relatives late in the evening"we expect more ridership and revenue."

NEW DELHI: The completion of Delhi Metro Rail Corporations Phase II lines by next year would result in a remarkable 60 per cent decline in man-hour losses for the working population of the National Capital Region, according to the findings of a new study by the Associated Chambers of Commerce and Industry (ASSOCHAM) of India.
Releasing the findings of the study here over the weekend, ASSOCHAM secretary-general D. S. Rawat said it is estimated that over 70 lakh people working in the region spent five hours daily commuting to work at present. The delays are caused by acute congestion on the crowded roads of Delhi and its satellite townships. However, once the Delhi Metro railway network spreads, travel time would be reduced by up to two hours for many.
Stating that most of the working population is expected to shift to Delhi Metro to avoid travel on congested roads, Mr. Rawat said the metro ridership is expected to shoot up from around 9 lakh per day at present to over 25 lakh per day by 2011.
To enhance the ridership further, the Chamber has suggested that the Delhi Metro authorities explore operating to far-off areas like Greater Noida, Dadri, Sonepat, Ballabhgarh, Palwal and Surajpur.
The expansion of the Delhi Metro network would also result in an increase in employment opportunities for skilled and unskilled workers. According to ASSOCHAM estimates, about 50,000 job opportunities would be created for engineers (electronics, mechanical, electrical), customer relations officers, contractors, technicians, station controllers, train controllers, vigilance staff, public relations personnel, security personnel and supporting staff.
The study also forecasts that apart from real estate, other sectors like retail, food and beverages, hospitality and multiplexes would also benefit because of the Delhi Metro expansion.
On revenue generation, the Chamber analysis revealed that Delhi Metros earnings would shoot up by over 190 per cent and net Rs.2,100 crore by the year 2011. The key factors driving this phenomenal growth would be commercial use of metro railway premises.

Lack of professional ethics led to mishap: Delhi Metro chief Holding the lack of professional ethics responsible for the Delhi [ Images ] Metro accident, its Managing Director E Shreedharan has asked technology institutions to inculcate national values through their curriculum.
There were deficiencies in design due to lack of ethics which caused the incident, he said, and emphasised the need for imparting values and ethics to students while they are learning.
"Should we not educate and warn the engineers and scientists coming out of the technical institutes that professional ethics and values should not be compromised for short-term business advantage," he said while addressing a convocation of Indian Institute of Technology in Mumbai [ Images ].
"The design of a cantilever pier is not a complex exercise and it was lack of professional ethics which led to the deficient design," Shreedharan said.
"I am making this suggestion based on my long professional experience and particularly in the context of the tragic accident that took place in Delhi Metro on July 12, when a cantilever pier under load collapsed, resulting in the death of seven workers and injuries to 16 others," he said.
A high-level technical committee came to the conclusion that the collapse was due to serious design deficiency, he said, adding the structure was designed by a well known structural consultancy and was being constructed by one of the best known civil contractors of the country.
Addressing the IIT students, the veteran civil engineer, who is also credited with the success of the Konkan Railway, said that in his long professional career of over 56 years, "I have learnt that to be successful in professional life one must have certain attitude and commitment to values."
The Delhi Metro MD advocated to the students that it was necessary to have reasonably good health throughout their life as health is wealth and asked them to have moderate, balanced food habits, adequate sleep and regular exercise.
"None of these costs any extra money nor will need a good deal of your time and this is only a habit you have to cultivate and make it part of your life," he said.
He said there are several challenges facing the nation including corruption and these are challenges that institutions like IIT have to face.

Close on the heels of technical discrepancies found in some pillars of Yamuna Bank-Noida Metro corridor, the DMRC today shifted its opening to September end. Earlier, it had to open for public by August end. A DMRC spokesperson said the delay was because of the ongoing testing of the trains.
